Zahnarztregister and Vorbereitungszeit: What Comes After Dental Approbation?

Your professional licence, preparatory work and statutory-care approvals are separate steps. Here is how to keep them clear.

Author: Dr. Gokul Krishna 23 October 2026

Does dental Approbation complete everything? No. Approbation is your professional licence. Entry in the Zahnarztregister is a separate application. The general federal rule requires Approbation and at least two years of qualifying Vorbereitungszeit, with a qualification-based exception. Register entry also does not replace the approval needed for a particular statutory-care role.

So once you receive Approbation, your next question is not simply whether a clinic will employ you. You also need to understand the position you are joining and which approval applies to that position.

01. Who this guide is for

This guide is for Indian BDS dentists approaching dental Approbation or planning their first role after receiving it. It is also useful when an employer mentions a Vorbereitungsassistent position and you want to understand where it fits.

It covers federal register requirements and a clearly labelled KZV Berlin administrative example. It is not a medical MBBS training guide. If you are still completing licensing steps, start with the dental Approbation pathway, then return here for the statutory-care stage.

02. Separate the four steps

First, Approbation answers the professional-licence question. The Zahnarztregister belongs to the framework for dental care under Germany's statutory health-insurance system. The Kassenzahnärztliche Vereinigung, usually shortened to KZV, handles that register.

StepWhat it answers
ApprobationDo you hold the professional licence to practise dentistry?
VorbereitungszeitHave you completed the qualifying preparatory work, unless the statutory exception applies?
ZahnarztregisterHas the KZV accepted your separate register application?
Role-specific approvalIs the assistant position, statutory-care employment or own contracted practice properly approved?

These are connected, but one document does not stand in for all four. In particular, the register is not another Approbation examination. It is also not permission to open a contracted practice simply because your name has been entered.

Official basis: §1 ZHG on professional licensing and §3 Zahnärzte-ZV on register requirements.

03. What counts as Vorbereitungszeit?

Vorbereitungszeit means a preparatory period. Under the general rule in §3, the minimum is two years. But the rule is not simply “spend two years anywhere in dentistry”. The setting, employment arrangement and evidence matter.

The federal provision requires at least six months as an assistant or qualifying substitute for one or more contracted statutory-care dentists. For the remaining period, it allows employed work in university dental clinics, hospital dental departments, dental departments of the public health service or Bundeswehr, or dental clinics.

There are details inside that rule. Up to three months of the contracted-dentist component can be replaced by the same duration in a university dental clinic or a Bundeswehr dental department. Substitute work has an additional condition: at least one preceding year of employed assistant work with a contracted dentist or work in the other institutions specified in §3(3).

Periods shorter than three weeks cannot be credited under that provision. Nor can the listed work be credited when performed alongside running your own practice. These details are why the job title alone is not enough.

Before you count a period: send the KZV the workplace type, exact dates, weekly hours, licence held and proposed role. Ask which part of §3 the work can satisfy and what confirmation you will need. This is a planning checklist, not an additional statutory application form.

Do not calculate a part-time completion date by looking only at the calendar. Ask the competent KZV how it will assess your hours, interruptions and combined periods. A written answer gives you something specific to plan around.

04. The exception is about qualifications, not a passport

Section 3(4) removes the preparatory-period requirement for dentists meeting its qualification conditions. It refers to a recognised training qualification acquired in an EU member state, an EEA state or a treaty-covered state, together with authorisation to practise. Read the complete wording of §3(4), not just a summary saying “EU dentists are exempt”.

For an Indian BDS dentist, receiving German Approbation does not, by itself, establish that this exception applies. Equally, nationality alone is not the complete legal test. Your actual qualification history matters.

If your history includes another European qualification or recognition decision, give the KZV the complete documents and ask for its assessment. Do not assume that working elsewhere in Europe, changing citizenship or holding German Approbation automatically turns your original qualification into the qualification described in §3(4).

The practical question is: “On the basis of these qualifications, do I meet the exception, and what evidence do you require?” That is more useful than asking whether all Indian dentists follow one identical route.

05. Berlin example: approval before preparatory employment

KZV Berlin states that preparatory-assistant employment needs its prior approval. Its application information lists the assistant application and Approbation certificate. The federal assistant-approval framework appears in §32 Zahnärzte-ZV.

Berlin also distinguishes a Vorbereitungsassistent from an assistant working with a Berufserlaubnis under §13 ZHG. Its guidance says a dentist with that temporary permit cannot work as a Vorbereitungsassistent in Berlin. A permitted supervised assistant role is a different category.

So do not treat time under Berufserlaubnis as automatically interchangeable with the preparatory work required for register entry. Check your actual dates and licence history with the KZV. For the earlier licensing stage, see our dental Berufserlaubnis guide.

06. Keep a record from the start

Alongside your employment documents, keep a simple record for the future register application. The point is to make each period understandable without having to reconstruct it after leaving a practice.

  1. Identify the role: record whether it is preparatory assistance, another assistant category or a later approved employment position.
  2. Keep the approval: save the decision covering the workplace, role and authorised period.
  3. Track the actual work: note start and end dates, weekly hours, changes and interruptions.
  4. Collect confirmation: ask the KZV which employer certificate or local confirmation form it expects.
  5. Check before changing jobs: ask whether a new approval or updated evidence is needed.

This record is an organisational suggestion, not a guarantee that every period will count. The KZV assesses the evidence against the applicable requirements. If a clinic offers a useful clinical role, that does not answer the separate register-credit question.

You can use our dentist job-application guide to prepare applications, while keeping this approval-and-evidence check alongside your job search.

A practical enquiry you can adapt

When you contact the register office, explain the decision you need help with. Sending a CV alone may leave the office without the details needed to answer your question. Use this suggested structure, filling in your actual information:

I hold an Indian BDS qualification and received German dental Approbation on [date]. I am considering a position at [practice or institution] from [date], working [hours] per week. The proposed role is [exact role]. Could you confirm the approval needed before starting, whether this work can contribute to Vorbereitungszeit under §3 Zahnärzte-ZV, and the evidence required for a later register application? My earlier dental work and licence dates are listed separately.

If you are asking about the exception instead, replace the work-credit question with a request to assess your qualification documents under §3(4). Keep those two questions separate so the reply tells you whether you need preparation at all and, if you do, how the proposed work fits.

Once you receive a reply, compare it with the employer's proposed position. If the reply concerns preparatory assistance but the contract names another role, clarify the difference before treating the documents as a complete plan.

Before leaving a workplace, check that your confirmation identifies you, the employer, the actual period and the agreed working arrangement. Ask the register office whether its own form is needed. It is easier to resolve a missing detail while the practice still has your employment information readily available.

07. Apply to the correct register

Section 4 links register entry to the admission district where you live. If you have no residence within the regulation's territorial scope, it provides a choice of register. Ask the register office to confirm responsibility before sending documents.

The federal provision specifically names your birth certificate, Approbation certificate and evidence of dental activity after Approbation. It also addresses document forms and situations where documents cannot be produced. Follow the office's current instructions rather than assuming an ordinary scan will be sufficient.

Berlin publishes a separate checklist and application with an activity-history annex. Its list includes qualification, identity and preparation evidence, plus documents relevant to individual circumstances. That does not mean every applicant needs a doctorate or specialist title.

Check §4 Zahnärzte-ZV and the KZV Berlin register page for the precise application requirements.

08. Register entry is not the final employment approval

Now separate assistant employment from employment as an angestellter Zahnarzt under §32b. The latter requires approval by the Zulassungsausschuss, the admission committee. Register entry does not replace that decision, and an employment contract does not replace it either.

Berlin's register guidance also distinguishes own contracted-practice admission from approved employment. Read §32b Zahnärzte-ZV with §95 SGB V for the statutory-care framework.

Before agreeing a start date, ask the employer which exact approval route applies, who submits the application and when the decision must be available. Keep residence and work-authorisation questions separate: a KZV approval is not an immigration decision.

09. India-specific planning and common mistakes

Your Indian internship and employment records can explain your professional background, but do not assume they automatically satisfy German preparatory requirements. Section 4 specifically asks for evidence of dental activity after Approbation. Ask the KZV about the legal basis for any period you want assessed.

  • Confusing a licence with register entry: keep a separate checklist for each.
  • Starting before approval is clear: settle the assistant category and approval with the employer first.
  • Counting only months: workplace type, hours and supporting certificates also need checking.
  • Assuming a nationality-based exemption: submit the actual qualification history.
  • Mixing preparation with specialisation: Vorbereitungszeit is not automatically a recognised dental specialty-training programme.

10. How MedGermany helps

MedGermany can help you organise the questions around your dental licensing stage, documents and next practical step. Bring your qualification history, licence status and proposed role so the discussion starts from your actual situation.

The KZV and the responsible approval bodies make their own decisions. Our role is to help you understand the pathway and prepare your questions, not promise that a particular period, application or job will be accepted.

11. Frequently asked questions

Does Approbation automatically put me in the Zahnarztregister?

No. Register entry requires a separate application to the competent KZV. Approbation is one requirement; qualifying preparation or the applicable statutory exception is another.

Must every Indian dentist complete exactly two more years?

Do not decide from nationality alone. The general rule is at least two years of qualifying preparation. The KZV needs to assess your qualification history, any applicable exception and the periods you want credited.

Can I count work under a dental Berufserlaubnis?

Do not count it automatically. Berlin distinguishes temporary-permit assistance from preparatory assistance and does not allow the latter with Berufserlaubnis. Ask your competent KZV to assess the exact role and dates.

Is preparatory assistance the same as a dental assistant job?

A Vorbereitungsassistent is a dentist undertaking qualifying preparatory work. It is not a generic label for every employee assisting in a dental practice. Ask for the precise professional role and approval category.

Can I open a contracted practice immediately after register entry?

Register entry is not itself admission to run a statutory-care practice. Own-practice admission and approved employment have further procedures. Clarify the intended route with the responsible KZV and admission committee.

12. Official sources and your next step

The official federal provisions and KZV Berlin guidance linked beside the relevant sections were checked on 23 October 2026. Federal requirements and the Berlin example are kept separate. For an individual decision, use your competent office's current forms and written assessment.
